Metastases are the most common malignancy to affect the spinal column. Bone marrow signal is uniform throughout the spine and is high signal relative to the intervertebral discs. These patterns include microscopic infiltration with normal MRI appearance in up to 20%, focal lesions, homogeneous diffuse infiltration of the bone marrow (best seen as low signal on T1WI), combined diffuse and focal disease, and a heterogeneous/variegated pattern with interposition of fat islands giving a salt and pepper appearance.41 Marrow involvement or lesions are typically hypointense on T1WI, hyperintense on T2WI and STIR images, and enhance avidly with gadolinium.

Normal bone marrow in adults is homogeneous and has high signal relative to the intervertebral discs. Summation artifact (superimposition of breast tissue): The DBT movies obtained at the diagnostic evaluation best demonstrated that this asymmetry is created 9-3 Typical benign hemangiomas found incidentally in a 70-year-old woman imaged for back pain. Vertebral body destruction and fractures are common with spine involvement.40 Punched out lytic bone lesions, diffuse osteopenia, fractures, and, rarely, sclerotic lesions are the hallmarks of disease on CT and radiographs.1 MRI findings reflect a number of different patterns of bone marrow involvement. Most lesions have a lytic appearance on radiographs and CT, but blastic or sclerotic metastases can occur, especially in the setting of prostate carcinoma.1,2 Bone scan traditionally has been used to screen for metastatic disease to the spine. A, Off-midline sagittal T1WI demonstrates heterogeneous mildly hyper-intense lesion within the T11 and T12 vertebral bodies with soft tissue encroaching on the T1112 neural foramen, obliterating the normal perineural fat in this location.

Cerebrospinal fluid (CSF) is dark.

Note the beam hardening artifact and streak artifact in the lower spine that are a result of metallic hardware. A, Sagittal CT image demonstrates diffuse mottled appearance of the spine because of numerous lytic lesions.

In children, neuroblastoma and Ewing sarcoma are the most common primary malignancies to metastasize to the spine.21,28 Metastases typically involve the vertebral body and posterior elements, and compression fractures as well as extension into the epidural space are common features. C, Sagittal post-gadolinium T1-weighted fat saturated image. A discussion of contraindications is beyond the scope of this chapter, but a useful reference is Shellocks Reference Manual for Magnetic Resonance Safety, Implants, and Devices.6 Of note, metallic implants for spinal fusion are not a contraindication, but may create magnetic susceptibility artifacts, which are greatest on fat saturated images and gradient echo (GRE) sequences and are minimized with the fast spin echo (FSE) technique.7 When assessing the spine, sagittal spin echo T1-weight images (T1WI) and T2-weighted images (T2WI) with axial GRE or FSE T2WI images are part of our routine protocol. Fig. Its still ongoing a study comparing the clinical performance of FFDM in two projections (CC + MLO) and those of DBT in a single projection (MLO) in compliance with dose constraint. Pacifici, Stefano. In the evaluation of neoplastic processes, post-gadolinium images and STIR (short tau inversion recovery, a fat saturated T2-weighted sequence) have been shown to increase sensitivity for detection of disease.810 Although some proponents advocate use of STIR instead of gadolinium-enhanced images for screening of vertebral body pathology,11,12 post-gadolinium fat saturated images may add clinically important information in cases of abnormal STIR images.13 Diffusion weighted imaging (DWI) has shown promise for differentiating benign from pathological fractures.1417 However, there are some reservations about DWIs specificity in this setting,18 and DWI has shown to be no better than conventional imaging in the absence of a fracture.19 For now, DWI remains unproven and technically challenging in the spine (Tables 9-1 and 9-2 and Figs.

Other radiographic artifact includes clothing or jewellery not removed. Findings ultimately shown to represent characteristically benign findings were recorded as summation artifacts or characteristically benign lesions (e.g., cysts and lymph nodes). The difference between best practices in hospital and forensic radiology may explain how this occurred. There is evidence of prior pelvic radiation with postradiation changes seen within L5 and the sacrum. C, Sagittal STIR.

Multiple myeloma is a multifocal malignant proliferation of monoclonal plasma cells that occurs most commonly in men older than 60 years. 75-year-old male with prostate cancer and diffuse metastatic disease throughout the spine.

CT provides useful diagnostic information, characterizing cortical destruction, lesion margins, and tumor matrix, and may demonstrate pathognomonic features for specific lesions. No other spinal lesions were seen. Motion artifact. Because they result from the perspective from which a particular view is taken, these supposed lesions disappear when the breast is viewed from another angle.

Hemangiomas are benign vascular tumors that occur in more than 10% of adults and are commonly detected as an incidental finding on imaging studies performed for unrelated indications.

Respiratory motion, often uncontrollable in the elderly, may cause great difficulty in the use of color Doppler ultrasonography. d) simple (a new method would require technicians and radiologists to learn new procedures for examination and assessment). C, Sagittal STIR image shows uniform low signal within the bone marrow with no evidence of bone marrow edema as would be seen with acute fractures or with an underlying lesion. MRI has been shown to be both more sensitive and specific than scintigraphy. 9-8 A 46-year-old man with known history of colon cancer. MRI of the thoracic spine demonstrates solitary T4 metastasis.

Of these, CT and MRI are relied on the most heavily. Webmographic evaluation has proved that the asymmetry identified at screening was a summation artifact (superimposition of normal breast structures) this, of course, assumes that the spot-com - pression/spot-compression magnification views were of diagnostic image quality, with the area of concern centered in the spot-compression paddle. 9-15 and 9-16).

While striving to detect Reconstruction algorithm in V-DBT system takes full advantage of any information provided in the 0 projection, which is basically a standard mammogram characterized by high contrast, which also provides valuable information for the microcalcifications visualization and their identification by 3D CAD, not yet available, but certainly among the future developments related to DBT. Hello, I Really need some help. Tomosynthesis, therefore, does not provide direct projection images, but reconstructed images of any individual layers through several available algorithms, more or less efficient, each aimed to remove from reconstructed slice the upper and lower layers "structured noise".

DBT allows the detection of a greater number of expansive lesions and a better morphological analysis of masses and architectural distortions, thanks to the contrast of findings greater than the background, given by the more shade structures belonging to the upper and lower layers, and then to the smaller amount of noise. C, Off-midline sagittal post-gadolinium T1-weighted fat saturated image demonstrates the heterogeneously enhancing hemangioma with an epidural component encroaching on the neural foramina above and below T9 and extension into the posterior elements. Typical benign hemangiomas found incidentally in a 70-year-old woman imaged for back pain. B, Sagittal T2WI demonstrates high signal within this lesion confirming the diagnosis of hemangioma. 2. E, Sagittal CT reconstruction. They are the most common primary bone tumor in adults, occur most commonly in the thoracic spine, and are usually solitary but can be multiple in approximately 30% of cases.1,2,21 They typically arise in the vertebral body but may involve the posterior elements.

A true lesion of this size could not have been obscured on this view. Bone marrow has high signal on this sequence, which may obscure some vertebral body lesions. The CT appearance of a low attenuation lesion with coarse trabeculae throughout (giving a polka-dot appearance in cross-section) is diagnostic.22 MRI demonstrates the fatty stroma, which is bright on T1WI and iso-intense to hyperintense to marrow on T2WI, with avid enhancement after administration of gadolinium.23 Bone scan is typically normal.24 An aggressive subtype of hemangioma is recognized that tends to be associated more commonly with epidural extension and pathological fracture. During the acquisition, any detector element receives in time sequence-related information on each object volume element.

Note that bone marrow signal on the T1-weighted sequence is slightly lower than that seen in examples of older patients, but is still greater than the adjacent intervertebral discs. If cortical disruption had not been present, this lesion would be easy to dismiss as a Schmorls node related to degenerative change. F, Axial post-gadolinium T1-weighted fat saturated image at the T12 level. The vertebral body components enhance heterogeneously and slightly less avidly.

B, Axial CT image demonstrates a focal lytic metastatic lesion within the posterior vertebral body with associated cortical disruption. B, Sagittal T2-weighted sequence.

There is also a small amount of epidural enhancing material encasing the thecal sac, which increases the likelihood that this represents a fracture caused by underlying malignancy. (2012) American Journal of Roentgenology. Table 9-2 Classification of Vertebral Body Lesions. Diffuse heterogeneous enhancement is present throughout the lumbar spine. The epidural component of the aggressive hemangioma, extending into the intervertebral neural foramen, enhances brightly and homogeneously. WebThe City of Fawn Creek is located in the State of Kansas. Vecchio S, Albanese A, Vignoli P et-al. Table 9-5 Imaging Features to Differentiate Benign Fracture from Malignancy. Fig. A, Sagittal T1WI. 9-1 and 9-2). Steven P. Poplack, Tor D. et al. Digital breast tomosynthesis (DBT)is an imaging technique that allows a volumetric reconstruction of the whole breast from a finite number of low-dose two-dimensional projections obtained by different X-ray tube angles, with a geometric principle very similar to that applied in stratigraphic technique. This does not change the diagnosis of hemangioma. 75-year-old female with chronic osteoporotic compression fractures. Spinal decompression and fixation were performed extending from T11 to L2, with associated artifact from the posterior metallic hardware. Artifact from the metallic hardware is worst on this sequence and interferes with fat saturation at the adjacent levels. Radiographs, radionuclide scintigraphy (most often bone scan), positron-emission tomography (PET), computed tomography (CT), and magnetic resonance imaging (MRI) are the imaging modalities available for evaluating lesions of the vertebrae. Under the BI-RADS lexicon 5, there are four types of asymmetries: The most common cause for an asymmetry on screening mammography is superimposition of normal breast tissue (summation artifact) 6.

There is no encroachment on the spinal canal. Detection and diagnosis of asymmetries is challenging because they often are subtle and appear similar to typical fibroglandular tissue in the breast. CSF is bright because of T2-weighting.
These lesions are often iso-intense to hypointense to marrow on T1WI and can be impossible to distinguish from a malignant lesion, such as a metastasis, on imaging (Table 9-3 and Figs. A small amount of enhancing epidural soft tissue is present, but there is no evidence of cord or nerve root compression. Normal marrow is low signal as a result of fat saturation making enhancing lesions more conspicuous. Check for errors and try again. Tomosntesis de geometra y dosis variable.

9-11 to 9-14). MRI vs. CT visualization of metastatic disease.

Normal horizontal linear enhancement can be seen centrally within the vertebral bodies as a result of the basivertebral venous plexus. C, Midline T2WI shows both the vertebral body lesions and epidural component to be hyper-intense to normal marrow and soft tissue structures.

E, Axial post-gadolinium T1-weighted fat saturated image at the L4 level demonstrates the enhancing fractured vertebral body. An interesting alternative is represented by variable geometry (V-DBT), which offers the highest 3D resolution at maximum speed acquisition due to a non-uniform sampling.

MRI is helpful for following treatment response, with decreased T2 signal abnormality and decreased enhancement representing good prognostic signs.42,43 Bone scan has limited sensitivity, detecting bone involvement in 75% of myeloma patients and only demonstrating 10% of lesions. Normal bone marrow is low signal with linear high signal present as a result of the basivertebral venous plexus. I, Axial CT image at the T6 level. C, Axial post-gadolinium T1-weighted fat saturated image demonstrates diffuse involvement of the vertebral body and cortical disruption posteriorly in the midline with epidural extension. A, Sagittal T1WI of the thoracic spine demonstrates a focal low intensity lesion within the right T6 pedicle. Radiographs, radionuclide scintigraphy (most often bone scan), positron-emission tomography (PET), computed tomography (CT), and magnetic resonance imaging (MRI) are the imaging modalities available for evaluating lesions of the vertebrae. One thousand eighty-six (53.7%) studies with one-view-only findings were judged to represent superimposition of normal breast structures (summation artifact) simply from the standard projections obtained at screening; findings in an additional 587 (29.0%) studies were characterized as representing superimposition of normal structures

However, the latter two solutions can result in lower image contrast, although this may not be clinically significant at digital mammography due to its higher image contrast compared with screen-film mammography. There is pathologic fracture through the L2 vertebral body with approximately 30% loss of vertebral body height. This artifact is caused by summation of overlapping tissues creating a pseudo mass. Differential diagnosis of vertebral body lesions can be narrowed by characterizing them as single or multiple. The reason we most often ask someone to return for additional views is because of a summation shadow. This occurs when several insignificant areas of dense tissue appear together in one location on a mammogram, creating a shadow that appears to be a major/significant density. Spinal decompression and fixation were performed extending from T11 to L2, with associated artifact from the posterior metallic hardware. C, Sagittal STIR image. clothing, external cardiac monitor leads, body parts of carer, etc. Mild endplate degenerative changes are present at L5S1. Bone marrow signal is otherwise uniform throughout the lumbar spine and normal for a young patient.
